After completing this course, participants will be able to:

  • Describe the pharmacology and history of articaine, including its development and introduction into clinical dentistry.
  • Compare and contrast the effectiveness of articaine with other commonly used local anesthetics in terms of potency, lipid solubility, duration of action, and safety.
  • Analyze the A, B, C Protocol (Articaine administration, Buffering, and Cold Testing) and its role in improving anesthetic efficacy.
  • Evaluate the clinical benefits of articaine for specific patient populations, including children, pregnant women, and geriatric patients.
  • Demonstrate appropriate techniques for administering articaine via infiltration and inferior alveolar nerve block (IANB), including strategies to enhance anesthesia success rates.
  • Identify best practices for minimizing patient discomfort and reducing the risk of soft tissue injuries associated with local anesthesia.
  • Assess the safety considerations of articaine, including its lower systemic toxicity risk and faster metabolism compared to other amide anesthetics.

Abstract

This course explores the advancements and applications of local anesthetics in dentistry, with a focus on articaine and its role in modern dental anesthesia. Participants will examine the pharmacological properties, efficacy, safety, and clinical benefits of articaine compared to other local anesthetics. The A, B, C Protocol, which enhances anesthetic effectiveness and patient comfort, will be analyzed in detail. The course also covers articaine’s unique properties, including its metabolism, half-life, lipid solubility, and its use in special patient populations (pediatric, geriatric, pregnant, and nursing patients). Emphasis will be placed on optimizing anesthetic success while minimizing complications, including local anesthetic systemic toxicity (LAST) and paresthesia risks.

Author Bio

Stanley F. Malamed, DDS

Dr. Malamed was born and raised in the Bronx, New York, graduating from the New York University College of Dentistry in 1969. He then completed a dental internship and residency in anesthesiology at Montefiore Hospital and Medical Center in the Bronx, New York, before serving for 2 years in the U.S. Army Dental Corps at Ft. Knox, Kentucky. In 1973, Dr. Malamed joined the faculty of the Herman Ostrow School of Dentistry of U.S.C., in Los Angeles, where today he is Professor of Anesthesia & Medicine. Dr. Malamed is a Diplomate of the American Dental Board of Anesthesiology, as well as a recipient of the Heidebrink Award [1996] from the American Dental Society of Anesthesiology and the Horace Wells Award from the International Federation of Dental Anesthesia Societies, 1997 (IFDAS). Dr. Malamed has authored more than 140 scientific papers and 17 chapters in various medical and dental journals and textbooks in the areas of physical evaluation, emergency medicine, local anesthesia, sedation, and general anesthesia. In his spare time, Dr. Malamed is an avid runner, exercise enthusiast, and admits an addiction to the New York Times crossword puzzle, which he has done daily since his freshman year in dental school.
