Educational Objectives
Upon completion of this continuing education course, participants will be able to:
Identify the clinical challenges and contraindications associated with treating patients with Lewy body disease and other neurological conditions, including aspiration risks, mobility limitations, and communication barriers that necessitate modified dental treatment approaches.
Describe the technical workflow and equipment requirements for implementing fast-sintering zirconia restorations in a single-visit protocol, including wireless scanning technology, chairside CAD/CAM milling, and rapid sintering protocols that reduce treatment time to under 90 minutes.
Evaluate the clinical adaptations and safety protocols necessary for treating special needs patients in modified clinical environments, including specialized isolation techniques, airway protection measures, and patient positioning considerations for medical transport beds.
Analyze the advantages and limitations of fast-sintering zirconia technology compared to traditional restorative materials and workflows, including patient selection criteria, practice integration requirements, and potential applications for other special needs populations
Abstract
This peer-reviewed clinical case report describes the successful treatment of a 75-year-old bedridden male patient with advanced Lewy body disease using a single-visit chairside zirconia restoration workflow. The patient required replacement of a fractured lithium disilicate crown on the mandibular right second molar (#31) while remaining in a medical transport bed throughout the procedure.
The treatment integrated wireless intraoral scanning, chairside CAD/CAM milling, and fast-sintering zirconia technology to complete the restoration in approximately 80–90 minutes from scan to delivery. Key adaptations included specialized isolation for airway protection, wireless scanning to improve ergonomics, and accelerated sintering protocols that reduced overall manufacturing time.
This approach demonstrates how modern digital dentistry can overcome significant barriers to care for medically compromised patients, eliminating the need for temporization, minimizing aspiration risks, reducing medical transport requirements, and enhancing patient safety through streamlined, single-visit protocols.

Dr. Juergen Langenbach
Dr. Langenbach is a practicing general dentist with nearly three decades of experience in digital dentistry and special needs patient care. He received his dental degree from Johannes Gutenberg University in Mainz, Germany, and has been implementing CAD/CAM technology in his practice since 2004, beginning with early chairside restoration systems.
His practice has developed specialized protocols for treating patients with neurological conditions and disabilities, working closely with medical professionals and anesthesiologists to provide comprehensive care. Drawing on time-tested techniques, digital dentistry, honed surgical skills, and an artist’s eye, he delivers predictable, stable, harmonious, and aesthetic results.
Emphasizing digital technology, Dr. Langenbach can explain to patients and peers the use of computerized dentistry in a way that removes fearful barriers. His passion is not only that of the student but also of the teacher. As philosopher and educator John Dewey said, “If we teach today’s students as we taught yesterday’s, we rob them of tomorrow.”
